CLAIMS - HOW TO FILE/REPORT A CLAIM

IMMEDIATE REPORTING OF ALL CLAIMS OR POTENTIAL CLAIMS IS CRITICAL. ALL LAWSUITS MUST BE REPORTED IMMEDIATELY DUE TO THE TIME LIMITS IMPOSED FOR RESPONSIVE ACTIONS.

All claim forms must be completed and be sure to report all other pertinent information in detail; Names, Addresses, and Telephone Numbers (both Home & Business) of all persons involved, as well as any witness information. Any evidence that may be necessary for inspection should be maintained and photos should always be taken before any repairs are started.

Claim forms may be submitted by mail or fax. We will be introducing electronic forms filing in the near future. If you have a severe claim, please call the claims department and provide us with information to immediately start our investigation. Reports that are phoned to the claims office must be followed up, as quickly as possible, with a written loss notice. Please write anywhere on the notice "Phoned in and the date called". These reports may come from members, agents, or claimants. A properly completed, signed, and dated loss notice can have important legal significance in the claims settlement process and in filing with the state.

When a loss occurs anxiety, anger, and frustration are normal emotions experienced by an employee or claimant. Contact is normally made within two working days acknowledging receipt of the claim and instructing the employee/claimant/member as to what must be done to assist with settlement of the loss.

Remembering the 5 W's of Loss Reporting will ensure that necessary information is obtained and reported to the claims office.

2. Where did the loss occur?
3. What actually happened?
4. Who was involved?
5. Witnesses?

All reasonable steps should be taken to prevent further damage and/or injury by making temporary repairs and properly securing any properties with open access to the public.
